Step-by-Step Enrollment in the VA CCN

Verify Your Credentials

  • Hold a valid state license and/or certification in a discipline approved for SUD treatment.
  • Eligible credentials include: LCSW, LPC, LMFT, CADC, PsyD, PhD, MD, DO, NP with SUD or behavioral health scope.
  • Maintain appropriate DEA registration for MAT-prescribing professionals.

  • Active license and certification documents.
  • Resume or Curriculum Vitae (CV).
  • Signed W-9 form.
  • Proof of malpractice insurance (if required).
  • Clinical treatment protocols or supervisory agreements (if applicable).

  • Upon approval, you will receive a contract detailing billing, service authorization, and documentation guidelines.
  • After signing, you may begin offering VA-authorized SUD services.
