This is one of the most common misconceptions in veteran healthcare: many veterans assume their VA healthcare enrollment covers their spouse and children. In most cases, it does not. VA healthcare is an individual benefit for the veteran. Your family members need separate coverage — but there are specific programs designed to help, depending on your situation.
VA Healthcare Does Not Cover Your Family (In Most Cases)
When you enroll in VA healthcare, the enrollment covers you — the veteran. Your spouse, children, and other dependents are not covered under your VA enrollment. They cannot receive care at VA facilities under your enrollment (with very limited exceptions like certain caregiver programs and newborn care for enrolled pregnant veterans).
Healthcare Options for Your Family
If You Are Active Duty or Recently Separated
Your family may be eligible for TRICARE, the military’s health insurance program. TRICARE coverage for dependents is tied to the service member’s active duty status or Transitional Assistance Management Program (TAMP) eligibility after separation. Contact TRICARE at tricare.mil for current eligibility information.
If You Have a Permanent and Total Service-Connected Condition
Your spouse and children may be eligible for CHAMPVA — the Civilian Health and Medical Program of the Department of Veterans Affairs. CHAMPVA provides healthcare coverage for eligible dependents of veterans with permanent and total service-connected conditions. See Blog 9 in this series for a detailed CHAMPVA guide.
If You Died or Were Killed in Service or Due to Service-Connected Conditions
Your surviving spouse and dependents may be eligible for CHAMPVA or TRICARE survivor benefits, depending on the circumstances. See Blog 10 in this series.
For All Other Situations
Your family’s healthcare options include employer-sponsored insurance (through your employer or your spouse’s employer), health insurance marketplace plans (healthcare.gov), Medicaid (for families who meet income eligibility), Children’s Health Insurance Program (CHIP) for children in families who earn too much for Medicaid but cannot afford private insurance, and the VA Dental Insurance Program (VADIP), which is available to CHAMPVA beneficiaries.
CHAMPVA: The Family Coverage Most Veterans Do Not Know About
While VA healthcare itself does not cover your family, CHAMPVA (Civilian Health and Medical Program of the Department of Veterans Affairs) provides healthcare coverage for the spouse, surviving spouse, and children of veterans who meet specific service-connected criteria. If you are a veteran who meets the VA service-connected criteria that qualify your family for CHAMPVA, your family members can receive comprehensive healthcare coverage including doctor visits, hospitalizations, prescriptions, mental health care, and preventive services. CHAMPVA is not insurance — it is a VA benefit program that shares the cost of healthcare with eligible beneficiaries. If you think your family might be eligible, contact the CHAMPVA office at 1-800-733-8387 or visit va.gov/champva to learn about eligibility requirements and the application process.
TRICARE for Military Families
If you are a veteran who retired from active duty with 20 or more years of service, your family may be eligible for TRICARE rather than CHAMPVA. TRICARE provides comprehensive healthcare coverage for retirees and their families through several plan options including TRICARE Prime (managed care with assigned primary care manager), TRICARE Select (self-managed care with more provider flexibility), and TRICARE For Life (supplemental coverage for Medicare-eligible retirees). Guard and Reserve members may also qualify for TRICARE Reserve Select. TRICARE coverage is separate from VA healthcare — you can use VA healthcare for yourself while your family uses TRICARE. If you are unsure which program your family qualifies for, contact the Defense Health Agency TRICARE information line or visit tricare.mil for eligibility determination tools.
Special Family Programs and Caregiver Support
While VA healthcare does not directly cover your family, the VA offers several family-oriented programs that provide indirect support. The VA Caregiver Support Program provides training, support services, and in some cases a monthly stipend to family members who serve as caregivers for veterans with serious injuries or illnesses. Family counseling is available through Vet Centers for families of combat veterans. The VA also offers bereavement counseling for family members of veterans who die while receiving VA care or whose death is service-connected. For military families with children, the VA can provide guidance on connecting with Exceptional Family Member Program resources, military family support organizations, and community-based family services. Contact your VA social worker or the Caregiver Support Line at 1-855-260-3274 to learn about family programs you may not know exist.
Veterans with dependent children should also explore whether their children qualify for coverage through the Children Health Insurance Program (CHIP) or Medicaid in their state. These programs provide comprehensive healthcare coverage for children in families that meet income guidelines, and eligibility is not affected by the veteran VA healthcare enrollment. Your state department of health or healthcare.gov can help you determine whether your children qualify for these programs, which can provide coverage that the VA healthcare system itself does not offer for dependents.
The bottom line: your family has healthcare options even though VA healthcare itself is for veterans only. Understanding which programs apply to your family situation ensures that everyone in your household has access to the coverage they need.
Special Programs for Military Families
Beyond CHAMPVA and TRICARE, several other programs support veteran families. The VA Caregiver Support Program provides training, support, and in some cases monthly stipends to family caregivers of veterans with serious injuries. Family counseling is available through Vet Centers for families of combat veterans at no cost. The VA also provides bereavement counseling for families of veterans whose deaths are service-connected. Military OneSource offers free counseling, financial planning, and family support services to all military-connected families. Contact your VA social worker or the Caregiver Support Line at 1-855-260-3274 to learn about programs that serve your family.
